In use by health jurisdictions serving over 8 million Canadians, Strata Resource Matching and E-Referral seamlessly matches the specific needs of post-acute care clients with the particular resources and availability of providers in the community. Alberta Health Services - Calgary Zone, for example, achieved an annual drop of 67% (from 120 to 40) in the number of acute bed clients waiting for appropriate community placement.
