Home care services enable individuals in need of ongoing care to continue living at home or in the community. These services are typically provided to elderly or disabled people, those with chronic diseases or people who require short-term medical care after a hospital stay.
Home care can range from providing medication reminders and assisting with personal hygiene to professional nursing care, occupational therapy or physiotherapy. Services can be delivered by PCT staff such as community nurses or outsourced to commercial or not-for-profit care agencies.
PathWays Home Care manages the requesting of home care services and routes requests to appropriate service providers. This online solution empowers users to create, modify or extend service plans. Requests are sent to service providers in real time. To streamline billing reconciliation, authorised hours of service are tabulated and compared against the service levels actually delivered. PathWays Home Care assists health and social care providers in analysing existing contracts with home care suppliers, helps manage unmet needs and supports future resource planning.
