Sub-acute care – often termed Rehab and Recovery – is in-patient care designed for a person with an acute illness, injury or where a disease has escalated. It is typically a kind of treatment rendered immediately after, or instead of, acute hospitalisation to address complex medical conditions or to administer a technically intricate treatment of a patient’s underlying long-term condition.
Sub-acute care requires the coordinated services of a multidisciplinary team – which can include physicians, nurses, and other relevant professional disciplines – that is trained to assess and manage these conditions and perform the necessary procedures.
PathWays Sub Acute/Rehab effectively matches specific clinical needs to available sub-acute streams of care. It ensures that the correct patient care plans and treatment regimens are followed as patients with complex clinical issues are discharged from acute care. At the same time, PathWays enables patients to remain on various waiting lists for follow-on care once stabilisation or treatment course is completed.
British clients using PathWays Sub Acute/Rehab™ include Calgary Health, Capital Health (Edmonton) and the Toronto Central LHIN.
